Requirements:
1. Clear xerox copy of any Government Issued Valid ID (ex. Driver's License, TIN, SSS, PRC, Postal id, Voter's id, Company id, Philhealth id w/ pic, E-card, etc.)
2. Clear xerox copy of Proof of Billing under your Name (not more than 60 days old). If not under your name, you have to get an authorization letter from the owner of the POB & a xerox copy of his id. You could also get an Original Barangay Clearance (not more than 30 days old) and use it as your POB.
3. Pay a 1-month advanced monthly fee
